BROOKLYN DAY HABILITATION & TRANSITIONS PROGRAM OFFICE

Human First’s New York City Day Habilitation and “Transitions” After School and Saturday program services are provided in our Brooklyn office, which opened in July 2008.

THE PRIDE CENTER

Opened in January 2016, The Pride Center in Farmingdale, New York, is a state-of-the-art facility that houses our Long Island Day Habilitation program, Medicaid and Non-Medicaid service coordination, Community Habilitation, B2H and TBI program. The Pride Center provides services to individuals and families living in Nassau and Suffolk Counties, and is located at:

QUEENS

Human First opened its first office in Queens in 2012. Recently renovated, this office provides day habilitation and after school programs, and is the headquarters of our Medicaid Service Coordinators. The site is located at:
