Medicaid Service Coordination

Medicaid Service Coordination, is a Medicaid State Plan provided by the Office for People with Developmental Disabilities (OPWDD). This program assists persons with developmental disabilities in gaining access to necessary services and supports appropriate to the needs of the individual. Qualified service coordinators use a person centered planning process to develop an Individualized Support Plan (ISP) with and for a person with a developmental or intellectual disability. This service emphasizes the individuals choice in determining their goals and the services they receive to meet those goals. Individualized services and supports, and consumer satisfaction are also key components to this program.

Who is eligible to receive MSC?
In order to receive MSC, a person must:

  • Have a documented diagnosis of a developmental disability;
  • Be enrolled in Medicaid;
  • Demonstrate a need for ongoing and comprehensive , rather than incidental, service coordination;
  • Choose to receive MSC or have consent given on behalf o fan authorized individual;
  • Live outside of an Intermediate Care Facility, Developmental Center, Psychiatric Hospital, Small Residential Unit, Nursing Facility or hospital, or other Medicaid funded setting that provides service coordination;
  • Not be enrolled in any other comprehensive Medicaid long-term service coordination program.
