COMMUNITY HABILITATION

Community Habilitation is designed for individuals who do not reside in a residence certified or operated by OPWDD. Community habilitation services may be delivered at any non-certified location, including the individual’s home.

These services target the enhancement of daily living activities, communication, socialization, and community inclusion. Essential supports include:

• Travel training
• Self-advocacy
• Informed choice skills
• Leisure skills
• Behavior management

The program is offered throughout New York City and Long Island.
