NON-MEDICAID SERVICE COORDINATION

Human First provides service coordination to individuals living in Brooklyn who do not have Medicaid benefits. Individuals receive access to a variety of support services, including respite, advocacy, vocational, recreational and home health aide services.

To quality for Non-Medicaid Service Coordination, individuals must be OPWDD-eligible and have initiated contact with OPWDD’s Front Door program in Brooklyn.
