COMMUNITY HABILITATION

Community Habilitation is a Medicaid-funded program operated under OPWDD to provide one-to-one services to people with intellectual/developmental disabilities living at their home (not residence certified or operated by OPWDD). Community Habilitation services develop or enhance the skills needed to live more independently in their homes and stay connected to their community. Supports include:

  • Help with activities of daily living
  • Engaging with families on issues & support related to their loved ones
  • Community & relationship building
  • Travel training
  • Self-advocacy
  • Informed choice skills
  • Behavior management

Human First’s Community Habilitation program serves people supported throughout New York City. To be enrolled, you must be Medicaid eligible/active and meet OPWDD’s eligibility requirements. For more information, please contact intake@humanfirst.org.
